To support our patients, we offer a Financial Hardship Discount Program (also known as a Sliding Fee Program). This program provides reduced rates for self-pay and out-of-network patients who qualify based on verified household income and family size using the most recent Federal Poverty Guidelines (FPG) and California thresholds.

Submit the completed application and proof of income (e.g., recent pay stubs, W-2, tax return, or self-declaration) to our billing department.
